The presence of anti-nuclear antibodies (ANA) occurs with high frequency in
systemic autoimmune diseases. When an autoimmune disorder is suspected the first
diagnostic step is the screening for ANAs. These antibodies, directed against
components of the cell nucleus and the cytoplasm, were first associated with
systemic lupus erythematosus (SLE), but the list of involved diseases has
widened because many rheumatic diseases are characterized by the presence of one
or more. The LIAISON® ANA Screen is used for the collective qualitative
determination of autoantibodies directed against the following antigens:
double-stranded DNA (dsDNA), RNP/Sm (70 kDa), SSA (Ro) (rich in 60 kDa), SS-B
(La), Scl-70, Jo-1, centromere (CENP-B) and mitochondria in human serum or
plasma, along with detection of sera positive for HEp-2 immunofluorescence
antibody test. This test is intended as an aid in the diagnosis of connective
tissue diseases (CTD), such as systemic lupus erythematosus (SLE), mixed
connective tissue disease (MCTD), Sjögren's syndrome (SS), systemic sclerosis
(Ssc), idiopatic polymyositis (PM), dermatomyositis (DM) and CREST syndrome. |

Tests for IgG and IgM cardiolipin antibodies are frequently ordered to help
determine the cause of an unexplained thrombotic episode, recurrent miscarriage,
or thrombocytopenia. Cardiolipin antibodies may be ordered along with lupus
anticoagulant testing to help investigate the cause of a prolonged PTT
(activated partial thromboplastin time), especially if clinical findings suggest

Antibodies against dsDNA belong to the group of Anti-Nuclear Antibodies (ANA),
which are directed against various structures of the cell nuclei. Antibodies to
dsDNA are a characteristic marker in patients with untreated active SLE –
occurring with a prevalence ranging from 40% to 80% – and correlate with the
onset of lupus nephritis. The presence of antibodies to dsDNA strongly indicates
SLE, but their absence does not rule out SLE in all cases. Low levels of
anti-dsDNA antibodies occur in other rheumatic diseases and, with a lower
frequency, in individuals asymptomatic for rheumatic disease. Several studies
report a correlation between disease activity and fluctuations in antibody
levels. Consequently, quantification of anti-dsDNA autoantibodies is of
paramount importance for clinical management of patients, e.g., for controlling
the course of the disease, for measuring disease activity (which is directly
related to circulating antibody titer), and for following up therapy outcome. |
