Triiodothyronine (T3) test helps to determine whether the thyroid is functioning properly. It is ordered primarily to help diagnose hyperthyroidism and may be ordered to help monitor the progress of a patient with a known thyroid disorder. The T3 test is usually ordered following an abnormal TSH and T4 test. Either the total T3 or the free T3 may be ordered. Since most of the T3 is bound to protein, the total T3 can be affected by protein levels and protein binding ability, but the free T3 is not. T3 testing may be ordered along with thyroid antibodies to help diagnose Graves’ disease, an autoimmune disorder that is the most common cause of hyperthyroidism. |

Total T4 test is used for the diagnose of hyper- and hypothyroidism but can be
affected by the amount of protein available in the blood to bind to the hormone.
The free T4 test, however is not affected by protein levels. Since free T4 is
the active form of thyroxine, it is thought by many to be a more accurate
reflection of thyroid hormone function and, in most cases, its use has replaced
that of the total T4. T4 is ordered along with or following a TSH test to
determine whether the thyroid hormone feedback system is functioning as it

Thyroxine-binding globulin (TBG) binds Thyroid hormone in circulation. It is one
of three proteins (along with transthyretin and albumin) responsible for
carrying the thyroid hormones thyroxine (T4) and 3,5,3’-triiodothyronine (T3) in
the bloodstream. Of these three proteins, TBG has the highest affinity for T4
and T3, but is present in the lowest concentration. Despite its low
concentration, TBG carries the majority of T4 in serum. Due to the very low
serum concentration of T4 & T3, TBG is rarely more than 25% saturated with its
ligand. Unlike transthyretin and albumin, TBG has a single binding site for
T4/T3. TBG tests are used in finding the reason for elevated or diminished
levels of thyroid hormone. |
