  ACTH  
     
  Adrenocorticotropic hormone or corticotrophin (ACTH) is a 39 amino acid polypeptide secreted by the anterior pituitary gland. It stimulates the adrenal cortex to secrete glucocorticoids like Cortisol and exerts some control over secretion of Aldosterone, the other major steroid hormone secreted by the adrenal cortex. Within the pituitary gland, ACTH is produced in a process that also generates several other hormones. The precursor of ACTH is a large protein called propiomelanocortin (POMC) synthesized and proteolytically cleaved into fragments that include also lipotropin (precursor of beta-endorphin and met-encephalin) and melanocyte-stimulating hormone (MSH). Plasma ACTH determination is useful in diagnosing disorders of the hypothalamic-pituitary-adrenal system.

  Changed patterns of serum Cortisol levels have been observed in connection with abnormal ACTH levels, clinical depression, psychological stress, and such physiological stressors as hypoglycemia, illness, fever, trauma, surgery, fear, pain, physical exertion or extremes of temperature.  
     
  A Cortisol test may be ordered when a patient has symptoms that suggest Cushing’s syndrome (obesity, muscle wasting, and muscle weakness) or Addison’s disease (weakness, fatigue, increased pigmentation, among others).

  DHEAs  
     
  The main indications for DHEAs assay are for the diagnosis of endocrine pathologies, especially those of adrenal origin:  
  Evaluation of puberty: DHEAs levels are low in case of delayed puberty and high in case of precocious puberty.  
  Evaluation of adrenal function: DHEAs levels are low in primary and secondary hypoadrenalism; conversely, DHEAs is the only androgen present in abnormally high concentrations in hirsute amenorrhoeic women in whom the syndrome is of adrenal origin, and in patients affected by congenital adrenal hyperplasia (CAH). DHEAs assay is suggested during dexamethasone suppression tests, because DHEAs levels remain low for 30-90 days.  
     
  Most patients affected by Cushing's disease (pituitary etiology) have moderately elevated levels of DHEAs. High levels of DHEAs are often found in polycystic ovary syndrome, showing that adrenal hyperandrogenism is fairly typical of this syndrome. Very high levels of DHEAs in women are suggestive of a hormone-secreting adrenal tumor. Since DHEAs levels reflect adrenal androgen production, the measurement of serum concentrations serve as an early marker for the onset of adrenarche. Serum DHEAs measurements are most commonly employed in the differential diagnosis of a virilized patient. Virilization is due to high levels of androgens (testosterone), which can induce balding (alopecia), amenorrhea and abnormal hair growth (hirsutism). Virilizing tumors of the ovary are associated with marked increases in serum testosterone, but near normal DHEAs levels. Virilizing tumors of the adrenals secrete very large amounts of potent androgens as well as DHEAs.
